MUST I APPLY IN PERSON?

You must always appear in person when applying for your first passport. A parent or legal guardian must personally appear and execute the application for a child less than 16 years of age. All applicants, regardless of age must appear in person to execute the passport application. A parental consent form is required for minors. All individuals, including newborns, are required to have a passport issued in their own name. The signature of both parents is required on all applications for minors under the age of sixteen if both parents can't appear, one parent may sign and submit a notarized statement of consent for the second parent.
